I, {name} , hereby release Gordon Green and Sacred Sword Martial Arts (hereafter SSMA), all parties involved with the teaching, organization, location, promotion, and execution of this course from any personal injury, emotional or bodily harm sustained or suffered from me during, arising out of, or as a result of any activity associated with SSMA. I further release said individuals and involved parties from all claims of liability for any property or valuables lost, mislaid, or stolen. I do not have, to my knowledge, any physical condition or disability that would preclude my participation in this program and understand that choosing to train while ill, injured, or disabled puts me at my own risk and I am responsible at all times for my decision to train or abstain. I understand the terms above and my assumed responsibility for my safety and wellbeing. Please initial the following: 

I understand that this course will involve some strenuous physical activity and physical contact, and may be hazardous and result in personal injury. It is my responsibility to know and honor my own limits. At any time, I am free to politely excuse myself, stand aside, and cease participation in any activity, whether it is for physical or other reasons, and it is my responsibility to communicate my concerns with the instructor.

I understand that, given the nature of training, there may be practice weapons used. I understand the need for such tools and props in the context of the material presented and agree to their use within the course. 

I understand that the training provided in this course will in no way guarantee that I will be impervious to an assault or attack of any kind. The above named parties and their employees and affiliates will not be held liable for any damages or injuries sustained in an actual fight, self-defense situation, etc. 

I understand that no insurance is offered to me through Sacred Sword Martial Arts as of the date signed, and that it is my responsibility to insure myself against injury or damage to property, others, and myself.

I have read the above contract and fully understand its contents, and I hereby accept and agree to the terms, conditions and provisions written here.
